All applicants will require to provide:

• 2 forms of Photographic ID, i.e. Passport / Driving License, Birth Certificate, Marriage Certificate
• 2 forms of Current Address, i.e. Utility Bill or Bank Statement
• PVG
• Payment of £59 for New PVG Application or £18 for a Scheme Update
• Wage slip or National Insurance Card for proof of NI
• Training Cert. (SVQ / Moving and Handling / BLS / First aid)
• 2 names and professional Addresses for Referees

In addition, nurses will require to provide:

• NMC Pin card (for trained vacancies)
• Statement of Entry
• Copy of Original Nursing Certificate
• OH Blood Levels
